How to Use These PDFs

1. Copy the PDF forms to your hard drive by right-clicking (Windows) or Control + click (Mac) & open with Adobe Reader only.
Filling out PDF forms within your browser's window or opening with Preview will not work properly. Please call with any questions.
2. Fill out only the form saved to your computer, then submit it to Spotlight.
